摘要(Abstract)：

目的评价伽玛刀治疗未能行手术的早期肝细胞肝癌(HCC)的疗效。方法对28例不宜行手术治疗或不愿行手术治疗的直径≤5 cm的HCC患者行伽玛刀治疗,以50%等剂量曲线覆盖计划靶区(planning target volume,PTV)100%,70%等剂量曲线覆盖PTV>85%为计划要求,处方剂量3.8~6.1 Gy/次,6~8次,3次/周,2-3周内完成。用Kaplan Meier分析生存率。结果完全缓解(CR)12例(42.8%),部分缓解(PR)10例(35.7%),无变化(NC)5例(17.9%),进展(PD)1例(3.6%)。总有效率(CR+PR)22例(78.6%)。6月、1、2和3年生存率分别为100.0%、92.9%、82.1%和77.3%。结论伽玛刀对早期肝细胞肝癌患者的治疗是安全有效的,不良反应较小。当患者不宜行手术或拒绝手术时,低分次、高剂量伽玛刀治疗是早期HCC局部治疗的好选择。
